Job summary

Stonehenge Therapeutic Community is seeking an Intensive Case Manager to join a 24-7 Crisis Stabilization program in the Guelph area. The role supports intake, case management, and care coordination for clients in crisis related to mental health and substance use, with on-call and weekend rotation.

The position requires a college diploma or university degree in a relevant field, at least one year of experience, and a valid Ontario driver’s license with vehicle insurance coverage.

Qualifications

• Completion of a College Diploma or University Degree relevant to mental health, addiction, or social services.
• Minimum one year of progressive experience in addictions or mental health; trauma-informed practice asset.
• Experience working with individuals in mental health or substance use crises.
• Experience in social service settings with knowledge of case management systems and planning techniques.
• Lived experience related to addictions or mental health asset.

Responsibilities

• Process and accept referrals from Guelph Police Services, Wellington County OPP, and local hospitals.
• Screen and assess referrals for program admittance.
• Conduct intake and enrollment activities.
• Obtain all consents and authorizations to ensure effective communication with providers.
• Develop and implement discharge plans including housing access and program discharges.
• Create individualized care plans with health care, entitlements, life skills, and community integration.
