Job summary

CMHA Thames Valley Addiction and Mental Health Services seeks two Part-Time Transitional Case Managers for the Addiction, Substance Use Disorder Service in London, Ontario. This frontline role provides crisis intervention, brief case management, and system navigation to reduce emergency department visits.

Qualifications include post-secondary education in addictions or related fields, PVSC within six months, and ability to travel.

Qualifications

• Post-secondary education in related field (e.g., BSW, MSW) and/or equivalent.
• Ability to travel and provide own transportation; valid driver’s license and adequate insurance.

Responsibilities

• Provide psychosocial care and crisis intervention to individuals accessing hospital services.
• Coordinate with internal programs and community supports to connect clients to addiction and mental health services.
• Conduct intake, assessment, and short-term case management with a focus on transitional linkage to CMHA TVAMHS and community resources.
• Maintain PVSC status and comply with agency policies and procedures.
• Engage in client advocacy, care planning, and discharge planning to support continuity of care.
