Position Summary:

Reporting to the Manager of Medicine, the Coordinator of Discharge Planning facilitates safe, efficient, and timely discharges within the in-patient Programs. This role focuses on coordinating patient flow from hospital, timely access to inpatient beds by reducing avoidable admission delays, ensures effective communication among team members and improving the overall patient experience.

Working collaboratively with interdisciplinary teams (physicians, nurses, allied health professionals, patient navigators) and external partners (Ontario Health at Home, Community Support Services) the coordinator ensures that discharge planning is timely, patient-centered, and aligns with best practices. The role promotes smooth care transitions by addressing patient needs, capacities, and preferences, while participating in and/or lead interdisciplinary rounds to enhance patient flow and ensure seamless, coordinated discharges.

This individual provides day-to-day support in operational and discharge processes, supports key performance indicators related to patient flow and decreasing length of stay, assists with data reporting to internal and Ministry stakeholders and supports quality improvement initiatives to streamline transitions in care. The Coordinator of Discharge Planning serves as a navigator and liaison, building strong relationships within the hospital teams, post-acute sites and community stakeholders to ensure high-quality, integrated care delivery. This individual will work closely with post-acute programs (ie rehab) to ensure appropriate and timely access and continue to improve the patient’s discharge experience.

Working in partnership with the Manager of Medicine and Manager of Allied Health, the Coordinator of Discharge Planning will provide input, feedback and support operationalize formulating and implementing policies, procedures and standards necessary to support excellence in discharge planning and patient flow.

Skills and Qualifications:

  • Bachelor’s Degree in a Health-Related Field Required; Master’s Degree Preferred.
  • Regulated Health Professional with a current and in good standing registration with a regulatory college (ie RN, OT, PT, SW, SLP, RD)
  • 3-5 Years of Clinical Experience in an Acute Care Hospital Setting
  • Demonstrated knowledge of relevant legislation, including the Health Care Consent Act, Public Hospitals Act, Ministry of Health and Long-term care act
  • Strong Understanding of patient flow principles, length of stay management and capacity planning.
  • Demonstrated commitment to excellence in patient care, equity and person-centered practices
  • Demonstrated knowledge of policy and procedure development and staff education implementation.
  • Demonstrated experience collaborating with interdisciplinary teams to complete, review and submit applications and related documentation for patient care transitions.
  • Strong understanding of Alternate Level of Care (ALC) designation criteria and related data collection and reporting processes.
  • Demonstrated experience collaborating with interdisciplinary teams in an acute care environment.
  • Excellent interpersonal and communication skills, with the ability to lead interdisciplinary discussions
  • Ability to function effectively in a fast-paced, high-pressure environment.
  • Strong problem-solving, decision-making and conflict resolution skills.
  • Highly organized and detail-orientated with excellent time management and prioritization skills
  • Skilled in data analysis and KPI monitoring, including use of Microsoft Office Suite tools
  • Ability to work both independently and collaboratively within a team
  • Flexibility and adaptability to the changing needs of an acute care setting
  • Demonstrates supportive emotional care to the patient and their family/significant others from all diverse backgrounds
  • Demonstrates emotional intelligence to navigate high-stress situations with empathy, self-awareness, and professionalism, ensuring compassionate care for patients and respectful collaboration with colleagues.
  • Ability to recognize and manage emotions— both personal and in others— Demonstrates trauma informed care to support a positive patient-centered care environment.
  • This is a non-union position, with no guaranteed fixed hours. Flexibility is required to meet operational needs.
  • Regular attendance and commitment to professional development is required.
  • May be required to attend off-site meetings with key stakeholders or participate in quality initiatives beyond program level.
