Job summary

Fraser Health is seeking a Coordinator to develop, plan, facilitate and coordinate an Aboriginal health program for Elders, incorporating community needs and cultural content.

Reporting to the Aboriginal Health Manager East, you will collaborate with Fraser Health departments and community partners to implement objectives, manage resources, and guide program staffing and reporting. This role emphasizes leadership, budgeting awareness, and continuous improvement in service delivery.

Qualifications

• Bachelor's degree in Health or Social Sciences, Business or related field.
• 3–5 years related experience with Elders and Indigenous community health.
• Knowledge of Indigenous cultures; experience facilitating groups and program development.
• Equivalent combination of education, training and experience.

Responsibilities

• Designs, develops, plans, facilitates and coordinates the program for virtual and physical programming for older adults.
• Facilitates community virtual and in-person presentations and workshops.
• Organizes workflow; develops work plans, determines priorities and resolves day-to-day problems.
• Participates in selection, hiring, and orientation of the program team members.
• Collaborates with Fraser Health departments, FNHA and partners to update goals and guidelines.
• Engages with community partners to ensure program aligns with needs and includes cultural content.
• Allocates resources and coordinates team scheduling for equitable service delivery.
• Creates and analyzes reports to inform quality improvement recommendations.
• Monitors expenditures and provides budgeting recommendations.
• Acts as a resource to care teams and stakeholders on program matters.
• Coordinates client case conferences to ensure needs are met.
• Maintains relationships with departments and community stakeholders.
• Attends committees and working groups related to the program.
• Performs other related duties as assigned.

The salary range for this position is CAD $32.43 - $46.62 / hour
Detailed Overview

Supporting the Vision, Values, Purpose and Commitments of Fraser Health including service delivery that is centered around patients/clients/residents and families:

Reporting to the Aboriginal Health Manager East, the Coordinator develops, plans, facilitates and coordinates the program to meet identified needs of the community it serves. The Coordinator collaborates with other programs across Fraser Health as well as community partners to ensure the development and implementation of the program is aligned with community needs.

Responsibilities

1. Designs, develops, plans, facilitates and coordinates the program for both virtual and physical programming for older adults.
2. Facilitates community virtual and in-person presentations and workshops.
3. Organizes workflow; develops, coordinates and implements work plans that support the objectives of the program; determines priorities; resolves day-to-day problems; recommends process changes for workflow and staffing levels.
4. Participates in the selection, hiring, and orientation of the program team members; identifies development and training opportunities for staff.
5. Collaborates with FH departments, FNHA and community partners to develop, implement, evaluate, and update program specific goals and objectives, and related guidelines.
6. Engages with community partners to ensure program is developed and implemented inline with community needs and includes cultural content.
7. Allocates resources and coordinates team scheduling of the program to ensure equitable service delivery.
8. Tracks and maintains record of program activities; develops, compiles, and analyzes reports that help inform quality improvement recommendations for program.
9. Assists with monitoring expenditures within the established parameters; provides recommendations regarding budgeting priorities for the program.
10. Acts as a resource to care teams and provides consultative advice to external and internal stakeholders as required on all matters related to the program.
11. Coordinates and participates in client case conferences to ensure the clients? needs are met.
12. Establishes and maintains an effective working relationship and ongoing communications with other departments, community agencies and key stakeholders.
13. Attends and participates on various committees and working groups with respect to the program.
14. Performs other related duties as assigned.

Qualifications

Education and Experience

Bachelor's degree in Health or Social Sciences, Business or related field. Three (3) to five (5) years related experience working with Elders and Indigenous community health. Knowledge of Indigenous cultures, experience facilitating groups and program development. Or an equivalent combination of education, training and experience.
