Detailed Overview

Supporting the Vision, Values, Purpose and Commitments of Fraser Health including service delivery that is centered around patients/clients/residents and families:

Reporting to the Director, Home Support and Community Services, the Manager is responsible for developing, implementing and evaluating Specialized Community Services Programs (SCSP) in Fraser Health; collaborates with community operational leaders for Home Support, Day Programs for Older Adults, Caregiver Support, and Respite across all Fraser Health (FH) community sites, including FH affiliated Respite and DPOA providers as well as other community partners in order to meet the needs of the SCSP client population. Provides regional oversight and support to ensure clinical quality care standards are maintained within Fraser Health including FH affiliated facilities. Leads regional internal and external meetings to engage stakeholders, identifies and resolves clinical issues, and ensures consistent delivery of safe and appropriate quality care. Collaborates with the Ministry of Health regarding reporting and SCSP policy and standards.

Responsibilities

1. Participates in the strategic planning process related to Specialized Community Services Programs (SCSP) across Fraser Health (FH); in collaboration with the Director, develops, implements and leads new initiatives; ensures consistency with regional and community objectives and operational plans, and compatibility with accepted standards of clinical practice and accreditation bodies.
2. Collaborates with community operational stakeholders to ensure coordination of service delivery and integrated practices across program areas; ensures clinical programs are developed to reflect best practice standards and guidelines.
3. In collaboration with the Director and other FH departments, provides business/system operational management of existing contracts for Respite and Day Programs for Older Adults (DPOA) providers across FH; negotiates operational plans with affiliated service providers, monitors performance, develops necessary action plans, and manages financial recoveries as required; develops mechanisms to track, analyze and report SCSP, Respite and DPOA contract utilization, budgets and targets.
4. Acts as a primary resource to clinical teams and relevant Home Support, Respite, DPOA and Caregiver Support clinical operations across FH. Ensures quality clinical standards are communicated and reviewed.
5. Recruits and manages assigned staff in accordance with professional standards, performance expectations and current legislated/contractual obligations; promotes and/or provides for co-operative working relationships, labour relations and staff mentoring and development in a continuous learning environment; co-ordinates the development, implementation and evaluation of staff training and orientation programs.
6. Prepares the annual operating and contract budgets for the assigned program area(s); monitors activities and ensures expenditures are within approved budgets; identifies efficiencies and promotes optimal use of resources, making recommendations for current and future program and service adjustments.
7. Authorizes supporting funding requests as required based on clinical or operational needs; responds to issues regarding clinical matters and supports compliance with the complaint review process.
8. Participates in regular site visits to review clinical indicators with service providers. Evaluates, documents and reports findings, and enables relationship building with providers during these processes. Represents FH during clinical audits that assess the delivery of services within FH affiliated providers and collaborates with relevant stakeholders through the auditing process.
9. Leads the evaluation processes that monitor service utilization, client and family experience and provider satisfaction. Works in collaboration with the Ministry of Health regarding reporting and home and community care policy.
10. Provides regional leadership including developing educational strategies and materials for clients/families, service providers, SCSP and the public as required, and monitoring web-based booking processes to ensure compliance and alignment with FH privacy standards and system reporting requirements.
11. Leads and chairs regional internal and external meetings for Fraser Health SCSP with community care teams and service providers aimed to discuss and resolve issues, within the terms of service contracts, and to ensure consistent delivery of service.
12. Participates in strategic planning, clinical services development, as well as the development of clinical and operational service plans, and helps lead various projects related to SCSP services.
13. Collaborates with clinical leaders, health care staff and service providers to foster an environment focused on quality client care. Leads and/or facilitates research and other quality improvement initiatives.

Qualifications

Education and Experience

Completion of an undergraduate degree in health sciences or related field, plus seven (7) years' recent related experience, including two (2) years in a leadership role; or an equivalent combination of education, training and experience.

Valid BC Driver's License and access to a personal vehicle for business-related purposes.
