Sage GroupHealthCare Staffing is seeking a Hospice Registered Nurse for a 13‑week travel/contract position in Lakeside, AZ. This full‑time role welcomes local and traveler candidates, with weekly gross pay from $2,080 to $2,120 and a travel/housing stipend for eligible travelers.

The RN will provide home‑based hospice care, create care plans, educate patients and families, and document visits within 24 hours.

Qualifications

• Registered Nurse with at least 1 year in hospice care
• Home care experience and HCHB charting proficiency
• Valid driver’s license and auto insurance
• Covid vaccination or medical/religious exemption
• ASN or higher required per job specs

Responsibilities

• Provide and coordinate care for hospice patients in their homes.
• Develop and implement nursing care plans including Start of Care and admissions.
• Support patient ability to stay at home and spend time with loved ones.
• Educate patients and families on disease management and care goals.
• Document charting for all visits within 24 hours and ensure accuracy.

Job Description:

• Provide Hospice nursingcare and services within the standards and scope of nursing practice asdefined within the state Nurse Practice Act regulations, policies, procedures,and protocols, as well as facility and corporate policies and procedures.
  
• Ensure physicians’orders are legible if taken verbally and transcribed appropriately perpolicy and procedure.
  
• Ensure patients’status is assessed on an ongoing basis and pertinent information isgathered and documented.
  
• Notify physicians ofsignificant changes in status.
  
• Document and completecharting for all patient visits within 24 hours.
  
• Provide bereavementsupport to patient’s families as needed.
