Job summary

Hopewell is seeking a dedicated Support Services Worker to join our on-site team in Guelph, ON. You will assist individuals with daily living, implement care plans, and support behavioural strategies within a multidisciplinary setting.

You will collaborate with a compassionate team, follow safety standards, and contribute to meaningful outcomes for residents through respectful, person-centred care.

Qualifications

• Experience with individuals with developmental and physical disabilities.
• Must have a valid class G driver’s license.
• University/college degree in a relevant field preferred.

Responsibilities

• Support individuals to live independently and engage in meaningful activities.
• Provide communication assistance and document interactions.
• Observe, assess, and report changes in behavior and abilities.
• Perform household tasks and personal care as per care plan.
• Follow Behaviour Support Plans and complete data collection.
• Assist with transportation for individuals.
• Maintain documentation and training compliance.
