Job summary

Prairie Mountain Health is seeking a Home Care Attendant to provide direct client care and environmental support in the home setting. You will help clients maintain independence and health while following established policies and procedures.

Key duties include assisting with activities of daily living, coordinating with a Case Coordinator, and communicating effectively within an interdisciplinary team. A Health Care Aide Certificate and Manitoba Class 5 license are preferred; other combinations

Qualifications

• Health Care Aide Certificate from a recognized Health Care Aide program.
• Other combinations of suitable education and experience may be considered.
• Manitoba Class 5 Drivers License or equivalent with access to a personal vehicle.
• Proficiency with Outlook, Word, Excel, Access and PowerPoint; internet applications.
• Demonstrated organizational skills and ability to work independently.
• Problem solving and decision-making skills.
• Flexibility to adapt to changes in techniques and procedures.
• Knowledge and competence of skills related to the position.
• Communication skills and ability to maintain confidentiality.
• Ability to perform duties regularly and respect diverse populations.

Responsibilities

• Demonstrates respect for client independence, dignity, privacy, and lifestyle at all times.
• Supports clients’ independence in activities of daily living, as assessed by the Case Coordinator.
• Adheres to safety and sanitation standards including hand-washing guidelines.
• May be assigned additional tasks per regulatory guidelines and supervision.
• Utilizes electronic programs effectively.
• Performs communication tasks (mail, email, photocopying, faxing, telephone) per PHIA and Privacy Act.
• Assists with meal preparation per Canada Food Guide or client diet.
• Organizes work according to client needs and service requests.
• Reports changes in client status to Supervisor/Case Coordinator per guidelines.
• Participates in staff meetings, case conferences, and in-service programs.
• Collaborates with co-workers to achieve client-centered goals.
• Other duties as assigned.
