Job summary

Mikisew Cree First Nation (MCFN) invites applications for a Casual Patient Navigator. The role supports patients and families through the healthcare journey, coordinating appointments, transportation, and access to community resources.

You will work with healthcare providers and community partners to ensure timely, compassionate care while maintaining confidentiality and professional standards across health programs and services.

Qualifications

  • High school diploma or equivalent is required.
  • Post-secondary education in Health Studies or related field is preferred.
  • Experience with First Nations communities or partners is required.
  • Proficiency in Microsoft Office applications.

Responsibilities

  • Provide compassionate, patient-centered support to members and their families throughout their healthcare journey.
  • Coordinate transportation arrangements for medical appointments with the Medical Transportation Program.
  • Assist patients with scheduling medical appointments and coordinating healthcare-related services.
  • Provide wellness visits, hospital visits, palliative support, and emotional support as required.
  • Facilitate communication between patients, families, healthcare providers, and support services.
  • Connect patients with NIHB and other community resources.
