Job summary

Pacific Coast Community Resources is hiring a Community Support Worker to provide care and support in staffed homes within the community. You will empower clients to live independently while ensuring high standards of care and safety.

You will collaborate with a multidisciplinary team to deliver person-centered services. The role emphasizes rights-based practice, daily living support, and documentation of outcomes in our care systems.

Qualifications

• Proven ability to communicate effectively with team members, professionals, families, and community members.
• Experience applying behaviour management principles and mental health supports.
• Ability to assist with activities of daily living and medication administration.
• Moderate computer literacy for case management entries.

Responsibilities

• Ensure the rights and choices of supported persons are respected and advocate as needed.
• Interact effectively with people we support.
• Empower individuals to be full citizens in their community.
• Develop, implement, and document Personal Service Plans.
• Foster relationships with family, professionals, and community members.
• Assist with daily living activities and medication administration.
• Record daily activity and health information in the case management system.
• Maintain home cleanliness and assist with meals and light housekeeping.
