Job summary

Pacific Coast Community Resources Inc is seeking a Community Inclusion Worker to support individuals in personal, social, vocational and volunteer activities in the community.

You will empower clients to live independently, collaborate with families and professionals, and document care plans and progress in our system. A focus on safety, rights, and inclusive community participation is essential to this role.

Qualifications

• Must have Grade 12 education and a related diploma
• Two years minimum experience in community social services/health care
• Current Emergency First Aid/CPR and Food Safe certificates
• Criminal record clearance for children and vulnerable adults
• Eligibility to work in Canada

Responsibilities

• Respect and advocate for the supported person’s rights and choices
• Interact effectively with people we support
• Empower the person to participate in community activities
• Coordinate and facilitate social, vocational, and volunteer activities
• Record daily activity and health information in the management system
• Maintain behavior management practices and mental health awareness
