Job summary

Pacific Coast Community Resources Inc. is hiring a Community Support Worker to join a dynamic team supporting individuals in staffed homes within the community.

You will promote independence, document care plans, and collaborate with a multidisciplinary team to ensure high standards of care. The role requires a Grade 12 education, a relevant diploma or equivalent, 2+ years in community health, and current First Aid/CPR and Food Safe certificates.

Qualifications

• Must have Grade 12 education and a Community Support Worker Diploma or equivalent.
• Minimum 2 years of experience in community social services/health care.
• Current Emergency First Aid/CPR and Food Safe certificates.
• Previous behaviour management training/courses.

Responsibilities

• Advocate for the rights and choices of people we support.
• Interact effectively with the people we support and their families.
• Empower people to be full citizens in their community.
• Develop, implement, and document Personal Service Plans.
• Maintain positive relationships with family, professionals, and staff.
• Assist with activities of daily living and medication administration.
• Record daily activity and health information in the case management database.
• Maintain a clean home and assist with light housekeeping and meals.

Job description

As our Community Support Worker, you will work as part of a vibrant team to provide care and support to people living in staffed homes within their community. You'll encourage and empower the people we support to be independent and self-determining and to live the life they envision while maintaining excellent standards of care, health, and safety. Your natural ability to care for others and work both independently and collaboratively as a member of a multidisciplinary team will allow you to influence your team's success.

Core Responsibilities

• Ensure the supported persons’ rights and choices are respected and advocate as needed
• Interact effectively with people we support
• Empower the people we support to be full citizens of their community
• Participate in the development, implementation, and documentation of Personal Service Plans
• Foster positive relationships with persons' family members, professionals, co-workers, and members of the community
• Assist with activities of daily living and medication administration
• Excellent understanding of behaviour management pri nciples and strategies including mental health needs
• Record the daily activity and personal health information (PHI) of the people we support on our case management database
• To work in conjunction with the people we support to ensure their home is clean and well maintained at all times, including daily food preparation and light housekeeping duties

You will have

• Grade 12 education
• Community Support Worker Diploma or equivalent
• Minimum 2 years of experience in community social services/health care
• Current Emergency First Aid/CPR and Food Safe certificates
• Previous behaviour management course/training
• Ability to effectively communicate (verbal & written) with team members, professionals, families, and community members
• Must be comfortable with full personal care
• Class 5 Driver's license. If driving a commercial company vehicle, a Class 4 license is required.
• Satisfactory Driver's Abstract
• Criminal record clearance for children and vulnerable adults
• Eligibility to work in Canada
