Job summary

ParaMed in Parry Sound, Ontario is seeking a Registered Practical Nurse to perform scheduled community visits, delivering ethical and safe nursing care within your scope. You will assess patient needs, implement and adjust care plans, and build rapport with diverse patients in their homes.

A driver’s license and ability to travel within the designated area are required, with emphasis on reliable, flexible, and compassionate care for vulnerable populations.

Qualifications

• Registered Practical Nurse with CNO in good standing
• Valid driver’s license and access to a vehicle required
• Experience delivering community care is an asset
• Reliability and flexibility are essential
• Willingness to travel to patient residences within a designated area
• Experience working with vulnerable populations
• Passion for making a difference in your community

Responsibilities

• Participate in scheduled visits in the community to look after patient needs within your scope of practice
• Provide ethical and safe nursing care
• Use appropriate assessment tools/techniques to assess patients’ individual needs
• Execute and continuously monitor care plans and adjust as required
• Engage patients using a collaborative approach by building rapport and trust
• Maintain and update professional patient records related to patient encounters and plans
