ABOUT THE ROLE

The CANDORA Society (CANDORA) is hiring a Community Resource Worker to support participants facing barriers to employment. If you are passionate about making a difference in the lives of individuals facing challenges such as housing instability, mental health, financial insecurity, childcare needs, and more, this is the role for you! As a case manager, you will work closely with Career Counsellors and other team members to ensure participants receive the wraparound support they need to move toward sustainable employment outcomes.

KEY RESPONSIBILITIES

Case Management and Wraparound Support

  • Conduct comprehensive assessments to identify participants' barriers to employment, including housing, mental health, financial instability, childcare, transportation, and criminal records.
  • Develop individualized support plans with participants, outlining steps to address barriers and connect them to community resources.
  • Act as a case manager, coordinating a network of community resources to provide holistic support.
  • Regularly update and revise Action Plans to reflect participants’ progress and changing needs.

Resource Coordination and Advocacy

  • Build strong relationships with community organizations, service providers, and agencies to ensure participants have access to the resources they need.
  • Advocate on behalf of participants to help them access housing, mental health services, financial support, childcare, transportation, and legal services.
  • Provide guidance and support to participants as they navigate complex systems such as housing applications and legal assistance.

Collaboration and Teamwork

  • Work closely with Career Counsellors to address non-employment-related barriers that affect participants' engagement in employment activities.
  • Participate in case conferences and team meetings to discuss participant progress and share resources.
  • Collaborate with community stakeholders to advocate for participants' needs and support their success.

Administrative and Program Tracking

  • Maintain accurate records of participant interactions, support plans, and progress in program databases.
  • Document all referrals, follow-ups, and outcomes to ensure accountability.
  • Assist in preparing reports on participant outcomes, resource utilization, and program impact.
  • Track and manage program expenses to ensure efficient use of resources.

Community Outreach and Engagement

  • Represent CANDORA at community events, networking opportunities, and partnership meetings to build relationships with service providers.
  • Identify gaps in community resources and advocate for the development of new services to meet unmet needs.
  • Participate in community development initiatives to enhance the availability of support services for participants.

Other Responsibilities

  • Provide information about CANDORA activities and upcoming programs.
  • Participate in relevant training and professional development opportunities.
  • Support CANDORA-wide initiatives and program-related events as required.

SKILLS, QUALIFICATIONS, AND COMPETENCIES

Education and Experience

  • Proven experience in case management, community resource coordination, or a similar role, particularly working with individuals facing barriers to employment.
  • Post-secondary education in Social Work, Community Development, Psychology, Human Services, or a related field. Equivalent education, training, and/or experience will also be considered.

Skills and Disposition

  • Strong knowledge of local community resources, including housing, mental health, financial assistance, childcare, and legal support services.
  • Experience in providing trauma-informed care and understanding the impact of trauma on engagement in employment and services.
  • Excellent interpersonal, communication, and advocacy skills, with the ability to build trust and rapport with diverse populations.
  • Experience in developing and maintaining support plans, conducting assessments, and updating Action Plans.
    Strong organizational skills and ability to maintain accurate records.
  • Fluency in additional languages is an asset.

Additional Requirements

  • Clear Police Information Check, including Vulnerable Sector, and Child and Youth Intervention Check (CYIM).
  • Applicants must have legal authorization to work in Canada.
  • Availability to work on-site, Monday to Friday.
  • Flexibility to work occasional evenings and weekends as required.

Employment Type: Permanent, full-time, up to 35 hours per week, dependent on program funding
