Job summary

Trulyalivefoundation seeks a Community Re-entry Program Coordinator – Justice Services to assist Black individuals transitioning from custody to the community across Saskatchewan. The role involves culturally responsive case management and coordinating reintegration plans with correctional and community partners.

You will work with correctional institutions and local organizations to promote safe community re-entry, reduce recidivism, and improve long-term outcomes for participants.

Responsibilities

• Provide culturally responsive case management and reintegration support to Black individuals transitioning from correctional custody into the community.
• Collaborate with correctional institutions, justice partners, and community organizations to develop individualized reintegration plans.
• Promote successful community adjustment and reduce recidivism through tailored supports.
