Job summary

Connective Support Society is seeking a dedicated outreach support worker to assist clients referred by funders, helping with adjustment to community life. The role requires crisis response experience, strong communication, and collaboration with the care team.

You will work 40 hours weekly, Tuesday to Saturday, with potential flexibility for program needs. A BC driver’s license and vehicle are required, and benefits align with the collective agreement.

Qualifications

• Diploma or Certificate in Social Services or a related field.
• Minimum of 1 year of related experience in social services.
• Ability to respond well in crisis situations.

Responsibilities

• Receives referrals from funders and assesses client needs.
• Develops an intervention and support plan with measurable goals.
• Addresses daily living needs (housing, nutrition, clothing, hygiene, etc.).
• Maintains records and progress notes per policy.
• Completes monthly progress reports and collaborates with the care team.
