Job summary

Connective Support Society is seeking an Outreach Support worker in Coquitlam, BC. The role focuses on enabling clients’ adjustment to life in the community and coordinating plans with the care team.

You will handle referrals, assess needs, and develop intervention plans, addressing housing, health, and access to services while collaborating with families and partners.

Qualifications

• This role requires a Diploma or Certificate in Social Services with about 1 year of relevant experience; in-house training may be provided.
• Experience serving clients with psychiatric involvement, developmental disabilities, or related needs is preferred.
• Knowledge of Lower Mainland and Fraser Valley community services is advantageous.

Responsibilities

• Receive referrals and assess client needs to determine intervention possibilities.
• Collaborate with clients, families, and professionals to develop measurable goals and plans.
• Address daily living needs including housing, nutrition, clothing, hygiene, and access to services.
• Ensure ongoing health and safety considerations are included in intervention plans.
• Provide interventions to help clients achieve established goals as needed.
• Maintain thorough case notes and participate in team meetings as required.

ROLE PURPOSE

This position will provide outreach support to client(s) referred to Connective by our funders. You will work to enable clients’ adjustment to and success with life in the community.

STATUS AND CLASSIFICATION

Unionized. This position is open to applicants of all genders.

HOURS

30 hours per week; Wednesday to Saturday, 1000 - 1730

LOCATION

Coquitlam, BC

REPORTING AND WORKING RELATIONSHIPS

• Reports and is accountable to the Program Manager or their designate.
• Participates as a member of Connective team to ensure quality and continuity of services to clients.
• Liaises with staff and volunteers of community and government agencies as required.
• Liaises with Connective staff as necessary.
• Accepts clinical direction from the care team.
• Participates as a member of Connective staff team.

KEY ACCOUNTABILITIES

• Receives referrals from funders.
• Works with the client, researches background material, and seeks the input of other professionals and family members, to provide the assessment that will ascertain needs and postulate intervention possibilities.
• Works with the client to consider issues and creates a mutually agreed intervention and support plan that has measurable short and long-term goals.
• Ensures issues of daily living, including but not limited to housing, nutrition, clothing, hygiene, leisure lifestyle and access, employment, and education are addressed in this plan.
• Ensures the client’s ongoing legal, mental, and physical health issues are addressed in the plan.
• When and as needed, provides a wide range of interventions to enable the client to address and accomplish the established goals.
• Interacts with other professionals, family members, and organizations to ensure services, admissions, and treatments are in the client’s interest and supportive of the long-term goals.
• On the client’s behalf, is a resource to other professionals, including but not limited to the courts, probation services, and community and institutional psychiatric services.
• Maintains extensive records and case notes in accord with Community Living British Columbia policies and standards.
• Completes monthly progress reports.
• Participates in staff and resident/staff meetings as instructed by the Program Manager.
• Other duties as subsequently negotiated.

WORKING CONDITIONS

• Functions independently, while remaining a crucial member of the care team.
• Adheres to Connective policies and procedures.
• Is available to work flexible hours as needed, which may include some evenings/weekends, as determined by program/client needs and schedules.
• Is a member of CUPE Local 1936-11.
• Receives benefits and works within the working conditions as outlined in the collective agreement and personnel manual.

EXPERIENCE AND SKILLS

• Graduate with a Diploma or Certificate in Social Services and one-year of relevant experience is preferred; inhouse training can be provided.
• Counselling experience in the appropriate field and demonstrate a clear ability to respond well in crisis situations.
• Experience with clients with a history of conflict with the law, developmental disabilities and/or spectrum disorders, interaction with social agencies and experiencing psychiatric/personality disorders.
• Understanding of providing service to people with different learning abilities.
• Extensive knowledge of community services in Lower Mainland and Fraser Valley as they relate to persons with psychiatric involvement.
• Experience in assessment, case/care plan development and implementation.
• Demonstrated an ability to deal with potential violence.
