Job summary

Life’s Journey Inc. in Winnipeg, MB invites applications for the Outreach Support Mentor role. This full-time, permanent position supports participants in the Outreach or Spectrum Connections program, reporting to the Program Manager.

Responsibilities include coordinating activities, maintaining program planning documents, and collaborating with the Outreach Team to ensure safety, quality, and compliance. Candidates should have previous outreach experience, the ability to work regular hours

Qualifications

• Previous outreach experience required.
• Two years’ related experience, preferably with individuals with disabilities.
• Experience in an outreach setting.
• Satisfactory background checks and driver’s license.
• First Aid Level 1 – Emergency certification.
• Reliable vehicle and valid insurance.

Responsibilities

• Provide direct support to program participants in the Outreach or Spectrum Connections programs.
• Assist in ongoing planning, monitoring, and communication of activities related to each participant’s plan.
• Follow Personnel Policies and Workplace Health and Safety policies while performing duties.
• Collaborate with Outreach Team, Case Workers, Clinical Case Managers, and the Program Manager.
• Ensure positive interactions with collaterals associated with participants.
