Job summary

Bayshore HealthCare in Windsor, Ontario seeks an Occupational Therapist to provide in-home assessment, treatment, education, and management for clients requiring Occupational Therapy. The role reports to Rehabilitation leadership and involves documentation, discharge planning, and collaboration with the health team.

The position requires a Masters in Occupational Therapy, registration with the Ontario College of OT, and experience with in-home services.

Qualifications

• Registered and in good standing with provincial College of Occupational Therapists.
• Experience with in-home assessments and treatments.

Responsibilities

• Conduct assessments, establish and revise treatment plans, set goals and discharge plans for clients requiring Occupational Therapy.
• Maintain accurate clinical records per College of Occupational Therapists of Ontario standards.
• Collaborate with the health team and educate clients/care providers on treatment strategies.
• Document results and progress; ensure confidentiality and compliance with Bayshore policies.

| Job Description | | | | --- | --- | | Community Occupational Therapist(Job Number: 26005665) Primary Location:ON-Windsor Clinical Employee Type:Contract Pay Rate From:Independent Contractor Pay Rate To:Independent Contractor Description : JOB SUMMARY The Occupational Therapist, under the direction of the Manager of Rehabilitation or Clinical Manager- Branch level, is responsible for the completion of professional activities that include: assessment, treatment, education, and management of clients requiring Occupational Therapy intervention. Clinical Responsibilities: • Assessment of clients, establishment and ongoing evaluation/revision of treatment programs, including goals and discharge plans. • Maintains current accurate clinical records according to standards set by College of Occupational Therapists. Document assessment results, goals, treatment, client progress and discharge plans. • Collaborates with other members of the health team as needed. • Communicates pertinent information and recommendations to clients/care providers and provides education regarding treatment strategies as needed. • Observes the performance of those assisting in the Occupational Therapy program and provides guidance as necessary. Other Responsibilities • Maintain active standing in College of Occupational Therapists of Ontario. • Participate in ongoing internal and/or external education activities • Adhere to Bayshore Healthcare / Bayshore Therapy & Rehab Policies and Procedures • Participate in quality activities and continuous improvement initiatives in keeping with the company's Quality Management System • Participate in proactive Health & Safety activities while performing all duties. Responsible to notify immediate Supervisor of any Health & Safety risks or concerns. • Maintain confidentiality of client and corporate information and discusses same only with appropriate Bayshore personnel • Complete other tasks as requested Qualifications : Education Masters Degree or recognized equivalent in Occupational Therapy. Registered and in good standing with provincial College of Occupational Therapists. Experience Experience with in-home assessments and treatments.
