Job summary

CarePartners is seeking RNs and RPNs to provide community nursing services in home and clinic settings in Ontario. Roles include flexible scheduling, workload variety, and opportunities to pursue specialization while maintaining patient dignity and privacy.

We offer full-time and part-time positions with on-call rotation. Applicants should be licensed in Ontario and capable of traveling as needed. Training and advancement opportunities are available.

Qualifications

• RN or RPN degree/diploma with good standing in the CNO.
• Current immunization record including 2-step TB test.
• Valid driver’s license and vehicle required for regular travel.
• Excellent English verbal and written communication skills.

Responsibilities

• Develop and maintain an effective Care Plan for Community Health Services, independently or with Ontario Health atHome.
• Provide nursing services to patients in-person and virtually.
• Deliver services in settings including in-home, clinic, retirement home or school.
• Communicate with patients, families and the care team.
• Perform other clinical duties as required.

Wages

• For Registered Nurses (RN)- This position offers a competitive compensation grid, with progression based on hours worked and role-specific criteria. The RN per general visit range is $30.20 to $44.58 and the RN hourly range is $30.80 to $45.48. Placement within the grid will be determined in accordance with company policies and procedures, as well as the candidate’s knowledge, skills, qualifications, and relevant experience.
• For Registered Practical Nurses (RPN) - This position offers a competitive compensation grid, with progression based on hours worked and role-specific criteria. The RPN per general visit range is $24.07 to $29.60 and the RPN hourly range is $24.07 to $29.60. Placement within the grid will be determined in accordance with company policies and procedures, as well as the candidate’s knowledge, skills, qualifications, and relevant experience.

What The Role Involves

• Developing and maintaining an effective Care Plan with respect to Community Health Services, independently or in cooperation with the Ontario Health atHome.
• Working with patients (both in-person and virtually) to provide exceptional nursing services
• Providing services in a variety of settings which may include in-home, clinic, retirement home or school
• Communicating and liaising with patients, family members and others within the interdisciplinary care team
• Performing other clinical duties as required

What You Bring

• Registered Nurse (RN) or Registered Practical Nurse (RPN) degree or diploma
• Current College of Nurses of Ontario (CNO) registration in good standing
• A strong desire and commitment to making a difference in the lives of our patients
• Exceptional interpersonal skills
• A valid driver’s license and vehicle as regular travel is required
• Excellent English verbal and written communication skills
• CPR Certification
• Current immunization record including 2 step TB test
