Job summary

Regent Park Community Health Centre in Toronto is seeking a Registered Nurse for primary care. The role involves direct patient care and community services in a high-density, multicultural neighborhood, with a focus on health promotion and education.

The RN works as part of a multidisciplinary team on-site and off-site, supporting vulnerable populations and addressing chronic conditions while advocating for social determinants of health and equity.

Qualifications

• Certification as a Registered Nurse in Ontario.
• Knowledge of health care system and social determinants of health.
• Experience in community primary care settings.
• Experience with electronic medical records (PS Suites an asset).

Responsibilities

• Triaging walk-in clients and telephone screening.
• Developing and implementing a care plan for chronic illness.
• Screening and assessing minor episodic illnesses.
• Providing counseling and crisis intervention.
• Referring clients to specialists or community resources as needed.
• Visiting clients at home when required.
• Performing basic nursing skills and assisting procedures.
• Assisting physicians with prescriptions and equipment procurement.
• Maintaining patient records and health promotion activities.
