Job summary

• Strong documentation and time-management abilities.
• Knowledge of RAAM Model is preferred.
• Must hold a valid driver's license and have access to a vehicle.

Responsibilities

• Provide patient-centered case management and support discharge planning to the community.
• Collaborate with multidisciplinary teams to coordinate care transitions.
• Engage families and clients using DBT-informed approaches and other therapies.
• Maintain thorough documentation and comply with privacy standards.
