The Community Liaison Nurse role with Vancouver Island Health Authority focuses on coordinating complex discharge planning, assessing client needs, and ensuring continuity of care across acute and community settings. You will work within a patient-centred model and collaborate with an inter-professional team to optimize safety and quality of care.

Reporting to Regional Clinical Operations, the position requires BD CNMs RN registration, gerontology experience, and a valid BC driver's license.

Qualifications

• Graduation from a recognized nursing program.
• Registration with the BC College of Nurses and Midwives (BCCNM) as a practicing RN registrant.
• Three (3) years' recent related experience in gerontology or equivalent mix of acute and community health.
• Valid Class 5 BC Driver's License.

Responsabilités

• Lead complex discharge planning across care settings including assessment, planning, organization and evaluation of care needs.
• Coordinate and document care in collaboration with clients, families and the inter-professional health care team.
• Liaise with client/families, primary care physicians, long-term care homes and community programs to promote continuity of care.
• Work in acute care and community settings, contributing to quality and safety improvements.
