Island Health is seeking a Community Health Worker who will provide essential personal care assistance to clients and families, promoting independence in a home care setting. The position requires a valid BC Driver's License, registration with the BC Care Aide Registry, and relevant healthcare certifications. Ideal candidates should demonstrate strong organizational and communication skills to work effectively in a multidisciplinary team.

As a member of a multidisciplinary healthcare team, the Community Health Worker provides personal care assistance and support to clients and their caregivers/families to promote and maximize independence in the home setting. This includes assistance with activities of daily living and delegated tasks, as directed and in accordance with established plans of care.
Travel may be required for this position. Transportation arrangements must meet the operational requirements of the Vancouver Island Health Authority and may require the use of a personal vehicle. A valid BC Driver's License is required.
Home Support/Resident Care Attendant/Health Care Assistant Certificate from a recognized post-secondary program, or an equivalent combination of education, training, and experience. Food Safe Certificate and a valid BC driver's license are also required.
Current registration and good standing with the BC Care Aide & Community Health Worker Registry.
