Une organisation de santé reconnue recherche un Community Health Worker pour rejoindre son équipe. Le rôle consiste à fournir une assistance personnelle et un soutien continus aux clients dans un cadre de soins à domicile. Le candidat idéal doit posséder une formation spécialisée et avoir d'excellentes compétences en communication, ainsi qu'une capacité à travailler de manière autonome. L'utilisation d'un véhicule personnel est requise pour ce poste.
In accordance with the Vision, Purpose, and Values, and strategic direction of the Vancouver Island Health Authority, patient safety is a priority and a shared responsibility. The requirement to continuously improve quality and safety is inherent in all aspects of this position.
As a member of a multidisciplinary healthcare team, the Community Health Worker provides personal care assistance and support to clients and their caregivers/families to promote and maximize independence in the home setting. This includes assistance with activities of daily living and delegated tasks, as directed and in accordance with established plans of care.
Travel may be required for this position. Transportation arrangements must meet the operational requirements of the Vancouver Island Health Authority and may necessitate the use of a personal vehicle. A valid BC Driver's License is required.
Home Support/Resident Care Attendant/Health Care Assistant Certificate from a recognized post-secondary program, or an equivalent combination of education, training, and experience. Food Safe Certificate and a valid BC driver's license are also required.
Current registration and good standing with the BC Care Aide & Community Health Worker Registry.
