Island Health is seeking a Community Health Worker to support clients in achieving independence through personal care assistance in a home setting. This role requires effective communication, organizational skills, and relevant certifications. The position may require travel, necessitating a valid BC Driver's License.
In accordance with the Vision, Purpose, and Values, and strategic direction of the Vancouver Island Health Authority, patient safety is a priority and a shared responsibility. Continuous improvement in quality and safety is integral to this position.
As a member of a multidisciplinary healthcare team, the Community Health Worker provides personal care assistance and support to clients and their caregivers/families to promote and maximize independence in the home setting. This includes assistance with activities of daily living and delegated tasks, as directed and in accordance with established plans of care.
Travel may be required for this position. Transportation arrangements must meet the operational requirements of Vancouver Island Health Authority and may involve the use of a personal vehicle. A valid BC Driver's License is required.
- Home Support/Resident Care Attendant/Health Care Assistant Certificate from a recognized post-secondary program, or an equivalent combination of education, training, and experience.
- Food Safe Certificate.
- Valid BC Driver's License.
- Current registration and good standing with the BC Care Aide & Community Health Worker Registry.
