As a member of a multidisciplinary healthcare team, the Community Health Worker provides personal care assistance and support to clients and their caregivers/families to promote and maximize independence in the home setting. This includes assistance with activities of daily living and delegated tasks, as directed and in accordance with established care plans.
Travel may be required for this position. Transportation arrangements must meet the operational requirements of the Vancouver Island Health Authority and may involve the use of a personal vehicle. A valid BC Driver's License is required.
