Job Description

In accordance with the Vision, Purpose, and Values, and strategic direction of the Vancouver Island Health Authority, patient safety is a priority and a shared responsibility. Continuous improvement in quality and safety is inherent in all aspects of this position.

As a member of a multidisciplinary healthcare team, the Community Health Worker provides personal care assistance and support to clients and their caregivers/families to promote independence in the home setting, including assistance with activities of daily living and delegated tasks, as directed and in accordance with established plans of care.

Travel may be required. Transportation arrangements must meet operational requirements and may include the use of a personal vehicle, for which a valid BC Driver's License is required.

Qualifications:

  • Home Support/Resident Care Attendant/Health Care Assistant Certificate from a recognized post-secondary program, or an equivalent combination of education, training, and experience.
  • Food Safe Certificate.
  • Valid BC Driver's License.
  • Current registration and in good standing with the BC Care Aide & Community Health Worker Registry.
