A leading healthcare provider in British Columbia is seeking a Community Health Worker to join their multidisciplinary team. The role involves providing personal care assistance and support to clients in their homes, promoting independence through daily living activities. As a member of a multidisciplinary healthcare team, the Community Health Worker provides personal care assistance and support to clients and their caregivers/families to promote independence in the home setting. This includes assistance with activities of daily living and delegated tasks, as directed and in accordance with established care plans.
Travel may be required, and transportation arrangements must meet operational needs. Use of a personal vehicle may be necessary, and a valid BC Driver's License is required.
Home Support/Resident Care Attendant/Health Care Assistant Certificate from a recognized post-secondary program, or an equivalent combination of education, training, and experience. Food Safe Certificate and a valid BC Driver's License.
Current registration and good standing with the BC Care Aide & Community Health Worker Registry.
