A leading healthcare provider in British Columbia is seeking a Community Health Worker to join their multidisciplinary team. The role involves providing personal care assistance to clients to promote independence at home. Candidates must possess relevant certifications and a valid BC driver's license. This position emphasizes patient safety and quality care.
In accordance with the Vision, Purpose, and Values, and strategic direction of the Vancouver Island Health Authority, patient safety is a priority and a shared responsibility. Continuous improvement of quality and safety is inherent in all aspects of this position.
As a member of a multidisciplinary healthcare team, the Community Health Worker provides personal care assistance and support to clients and their caregivers/families to promote and maximize independence in the home setting. This includes assistance with activities of daily living and delegated tasks, as directed and in accordance with established plans of care.
Travel may be required for this position. Transportation arrangements must meet the operational requirements of Vancouver Island Health Authority and may require the use of a personal vehicle. A valid BC Driver's License is required.
Home Support/Resident Care Attendant/Health Care Assistant Certificate from a recognized post-secondary program, or an equivalent combination of education, training, and experience. Food Safe Certificate and a valid BC driver's license.
Current registration and good standing with the BC Care Aide & Community Health Worker Registry.
