Job Description: Community Health Worker

In accordance with the Vision, Purpose, and Values, and strategic direction of the Vancouver Island Health Authority, patient safety is a priority and a shared responsibility. Continuous improvement in quality and safety is inherent in this position.

As part of a multidisciplinary healthcare team, the Community Health Worker provides personal care assistance and support to clients and their caregivers/families to promote independence at home. This includes assistance with activities of daily living and delegated tasks, as directed and in accordance with established care plans.

Travel may be required. Transportation arrangements must meet operational needs and may involve the use of a personal vehicle, for which a valid BC Driver's License is required.

Qualifications: Education, Training, and Experience

• Home Support/Resident Care Attendant/Health Care Assistant Certificate from a recognized post-secondary program, or an equivalent combination of education, training, and experience.
• Food Safe Certificate.
• Valid BC Driver's License.
• Current registration and good standing with the BC Care Aide & Community Health Worker Registry.
