Job summary

Alliance for Healthier Communities seeks a Community Health Worker focused on Preventative Care for Black, African and Caribbean communities in Peel Region. You will deliver culturally responsive health prevention activities and connect clients to wraparound health and social supports.

You will plan outreach, coordinate referrals, and maintain records in an EMR, working with a diverse team to improve access and equity. This role emphasizes upstream care and community engagement.

Qualifications

• Post-secondary diploma or degree in health promotion, social services, community development, public health or related field, or equivalent experience.
• Experience working with Black, African and Caribbean communities is required or strongly preferred.
• Experience with group facilitation and community outreach is important.
• Experience with service navigation, referrals and coordinating community resources.
• Ability to collect program data and contribute to funder reporting.
• Strong attention to detail and ability to manage competing priorities and deadlines.
• Knowledge of Peel region and willingness to learn new software quickly.

Responsibilities

LAMP Community Health Centre is a multi-service, community based charitable organization that provides a wide range of programs and services to improve a person's health. The Black Health and Social Services (BHSS Hub) is an Afrocentric, culturally-affirming hub offering primary health care, social services and mental health and addictions support to Black, African and Caribbean (BAC) communities in Peel Region. Roots Community Services, LAMP Community Health Centre and the Canadian Mental Health Association (CMHA) Peel Dufferin are partnering to offer this initiative which was being designed with the holistic health of the individual as the primary focus. This ensures that services are easily accessible and closer to home for Black, African and Caribbean (BAC) communities in Peel.

The Black Health and Social Services Hub is seeking a Community Health Worker focused on Preventative Care for Black, African and Caribbean (BAC) Communities living in Peel Region. This position is focused on providing upstream and proactive health care for individuals and communities through locally informed, culturally responsive interventions to improve access to health and wraparound, health and social service supports. This position is reporting to the Manager, BHSS Hub.

Primary Responsibilities

Plan and deliver activities for BAC communities, including community group programming, to support the development, implementation, and evaluation of health prevention initiatives.

• Maintain an understanding of the health and social service needs of BAC community members through data and research, and utilize this understanding to inform the planning and delivery of group programs for BAC communities.
• Conduct non-clinical needs screening and support clients in identifying appropriate health and social-service referrals.
• Conduct outreach across the Region of Peel, which includes attending community events, and forming relationships with organizations that provide care to BAC community members.
• Work in collaboration with the BHSS Hub team to identify resources (both internal to the Hub, in Peel Region, as well as within the Greater Toronto Area that will enhance BAC community member capacity to meet their goals and self-manage health needs.
• Attends stakeholder meetings where intake and service navigation matters are addressed, providing solutions and following up as appropriate in a timely manner with all concerned parties and nurtures those relationships as a trusted representative of LAMP CHC and the BHSS Hub.
• Contribute to overall team functioning by assisting where needed and with the development of quality and process improvement initiatives.
• Advance the Hub’s work by participation in activities that seek to strengthen collaborative and interdisciplinary teamwork, such as interdisciplinary committees.
• Maintain accurate client records in the EMR, ensuring consent, privacy and confidentiality requirements are met.
• Track participation, referrals and program outcomes, and contribute to evaluation and funder reporting.
• All other tasks and duties as assigned by the Supervisor/ Manager

2+ years of relevant experience in community healthcare, health education and program development.

• Post-secondary diploma or degree in health promotion, social services, community development, public health or a related field, or an equivalent combination of education and experience.
• Demonstrated experience working with Black, African and Caribbean communities as required or strongly preferred.
• Experience working with high priority and communities experiencing barriers to health and social services
• Demonstrated experience with group facilitation and community outreach experience.
• Demonstrated experience with service navigation, referrals and community resource coordination.
• Ability to collect program data and contribute to funder reporting.
• Strong attention to detail, with demonstrated ability to manage competing priorities, meet deadlines, take accountability for results and remain professional under pressure.
• Demonstrated reliability, initiative and ability to organize work independently while contributing effectively within an interdisciplinary team.
• Demonstrated ability to receive and apply feedback, learn new processes and adapt to changing operational requirements.
• Knowledge of the Peel region, and the surrounding community areas is preferred
• A second language that is reflective of the community being served is an asset.
• Ability to learn new software quickly and willingness to continuously develop technology skills.
