Job summary

ParaMed in Windsor, Ontario is seeking a Clinical Unregulated Care Provider Supervisor to oversee PSWs and HSWs delivering home health care. The role ensures safe, quality, ethical care and adept clinical interventions across community settings.

You will lead teams, conduct regular huddles, address concerns, and support care planning, documentation, and funder reporting. RN or RPN registration with 5+ years of practice and strong leadership are required.

Qualifications

• RN or RPN registration with the College of Nurses of Ontario (CCNO)
• Minimum of 5 years’ experience as a practicing Registered Nurse or Registered Practical Nurse
• Comprehensive patient health assessment skills are required
• Understanding of principles of community nursing practice
• Proven experience leading and managing a team
• Knowledge of clinical documentation standards and practice guidelines is required
• Excellent organizational, interpersonal, and teaching skills
• Commitment to professional development in clinical practice relevant to home health care
• Effective critical thinking and problem-solving skills
• Valid driver’s license and access to reliable transportation
• Able to provide a current, clear Vulnerable Sector Screening (VSS)
• C.N.A. Certification as a Community Health Nurse (CCHN(C)) is asset

Responsibilities

• Supervise and support PSW’s and HSW’s to ensure practices comply with legislated standards and company policies
• Respond to and resolve concerns from community staff, patients/families, and other team members
• Provide ongoing leadership through regular huddles, face-to-face meetings and one-on-ones
• Initiate patient safety incident investigations and ensure timelines and action plans are met
• Collaborate with nursing team to support clinical documentation and reporting per funder guidelines
• Engage patients through problem solving, health teaching, rapport building, and goal identification
• Engage staff in problem solving, providing education, coaching, and mentoring
• Maintain knowledge, skills, and attitudes to provide safe, efficient service and stay current with practice changes
