Job summary

ʔaq̓am is seeking a Community Health Nurse based in Cranbrook, BC. You will support the health and wellbeing of ʔaq̓amnik̓ and the ʔaq̓am Community through nursing care, case management, and health promotion that respects cultural values and community priorities.

In this role you will lead home and community care services, supervise a small team of Registered Care Aides and a Home Support Worker, and coordinate wellness programs in partnership with regional health agencies.

Qualifications

• 3–5+ years of experience in a community or clinical nursing role.
• Registered and in good standing with the BC College of Nursing Professionals (BCCNP).
• Valid Class 5 Driver’s License with clean driving record.
• Access to reliable transportation to travel within the ʔaq̓am Community and Cranbrook area.
• Ability to undergo Criminal Record and Vulnerable Sector Check.
• Valid CPR-C and First Aid certification.
• FoodSafe Level 1 certification (or ability to obtain on hire).

Responsibilities

• Conduct health assessments, develop care plans, and provide nursing services in homes and community settings.
• Coordinate and monitor home and community care programs to meet client needs.
• Supervise, mentor, and schedule Registered Care Aides and a Home Support Worker.
• Collaborate with FNHA, Ktunaxa Nation Council, Interior Health and partners to align services.
• Advocate for clients to access FNHA Blue Cross and other benefits.
• Manage medical supplies, immunizations, vaccines, and related equipment.
• Maintain accurate client records using Mustimuhw and Panorama.
• Support mental health referrals and culturally safe care resources.
• Develop and implement health and wellness initiatives and grant proposals.
