Job summary

Vancouver Coastal Health's Squamish Health Unit in Squamish, BC is seeking a Community Health Nurse to collaborate with an interdisciplinary team and coordinate care for clients with acute, chronic and palliative needs.

You will assess, plan, implement and evaluate home and community-based services, document in EMR, and support clients and families to navigate the health system while upholding BCCNM standards and VCH values.

Qualifications

• Current practicing registration as a Registered Nurse with the BC College of Nurses and Midwives (BCCNM).
• One year recent related clinical experience in a relevant clinical area working with complex clients/patients with chronic health conditions and/or functional mobility issues.
• Valid BC Driver’s License required as local area travel may require the use of a vehicle or other accepted mode of transportation.

Responsibilities

• Assess clients in home/community settings and develop care plans.
• Coordinate planning, implementation andEvaluation of nursing and other services with clients and families.
• Collaborate with an interdisciplinary team to ensure safe, effective transitions through care continuums.
