Mennonite New Life Centre of Toronto (MNLC) is seeking a Community Health Navigator and Coordinator to join the Community Health Services team. This role focuses on helping clients access healthcare and community supports through navigation and coordination with partners.

Ideal candidates are people-focused, organized, and compassionate, with the ability to connect individuals to the right resources and navigate complex systems within diverse communities.

• Experience in community services, healthcare navigation, settlement, client support.
• Experience interpreting or working with diverse communities.

Responsibilities

• Directly engage with clients, healthcare partners and community organizations to improve access to health and community services.
• Collaborate with the CHS team to coordinate navigation, support referrals and connect clients to appropriate resources.
