Job summary

South-East Ottawa Community Health Centre in Ottawa, Ontario, is seeking a Nurse Navigator to support clients in accessing health and social services within the community and broader health system. You will work with an interprofessional team to provide client-centred care coordination, health education, patient advocacy and system navigation to improve outcomes and reduce barriers for diverse populations.

ROLE OVERVIEW

Job Summary

The Nurse Navigator plays a key role in supporting clients to access, understand, and navigate health and social services within the community and broader healthcare system. Working as part of an interdisciplinary team, the Nurse Navigator provides client-centered care coordination, health education, patient
advocacy, and system navigation services, with a focus on improving health outcomes and reducing barriers to care for diverse and underserved populations.
The Nurse Navigator supports clients, including self-referred and unattached individuals, in accessing appropriate primary care, community-based services, and specialized health resources, helping ensure equitable access to care and improved health outcomes.

The Nurse Navigator works collaboratively with primary care providers, community agencies, hospitals, specialists, and social service organizations to ensure integrated and equitable care for clients and families.

POSITION RESPONSIBILITIES

Navigate and coordinate care by supporting clients to access and move through healthcare, social services, and community resources, coordinating referrals, follow-up, and transitions to promote continuity and positive health outcomes.

Assess client needs and coordinate individualized care by conducting comprehensive nursing assessments and developing, implementing, and evaluating care plans in partnership with clients, families, caregivers, and interdisciplinary team members.

Identify and reduce barriers to care by recognizing systemic, social, cultural, linguistic, financial, and other barriers that may impact access to services, and working collaboratively with clients and partners to develop accessible, equitable, and practical solutions. This includes a demonstrated ability to conduct mental health assessments, recognize signs of mental health crisis, provide brief supportive counselling, and coordinate access to appropriate mental health and addictions resources.

Provide health education and self-management support using evidence-informed approaches that promote health literacy, informed decision-making, client autonomy, and active participation in care, while respecting diverse backgrounds, experiences, identities, and preferences.

Build collaborative partnerships with physicians, nurse practitioners, specialists, allied health professionals, community organizations, and other service providers to facilitate coordinated, integrated, and culturally responsive care.

Contribute to quality and service improvement through participation in case conferences, care planning, quality improvement, program development, and other initiatives, while maintaining accurate, timely, confidential documentation in accordance with professional standards, organizational policies, and applicable legislation.

WHAT YOU BRING

• Bachelor of Science in Nursing (BScN).
• Current registration in good standing with the College of Nurses of Ontario (CNO) as a Registered Nurse (RN).

• Minimum two years of nursing experience in community health, primary care, hospital, public health, or related healthcare settings.

• Strong knowledge of community resources, healthcare systems, and social determinants of health.
• Excellent assessment, care coordination, and care planning skills.
• Exceptional communication, advocacy, and relationship-building abilities.
• Experience working with patients/clients experiencing mental health incidents and/or chronic struggles.
• Strong organizational, problem-solving, and case management skills.
• Ability to work independently and collaboratively within an interdisciplinary team.
• Demonstrated commitment to health equity, inclusion, anti-oppression, and culturally safe care.
• Proficiency with electronic medical records and standard computer applications.
• Strong interpersonal (oral and written) and problem-solving skills; a commitment to teamwork.

Salary

$73,746/yr - $86,759/yr
