Qualifications

  • Bachelor of Social Work from an accredited University (clinical degree)
  • Masters of Social Work from an accredited University (clinical degree) preferred
  • Registration with the Ontario College of Social Workers and Social Service Workers required
  • Membership with the Ontario Association of Social Workers preferred
  • 2 years of acute care hospital social work experience preferred
  • Excellent interpersonal and communication skills
  • Ability to work under pressure, prioritize demands and meet deadlines
  • Strong organizational and time management skills; adaptable to change

Responsibilities

  • Maintains knowledge of relevant legislation (Mental Health Act, Health Care Consent Act, Substitute Decisions Act, Public Hospitals Act, Long Term Care Act, PHIPA, Excellent Care for All Act)
  • Maintains expertise in generic social work practice and implements evidence-based practices including case management, crisis intervention and discharge planning
  • Conducts comprehensive psychosocial assessments from a systems perspective including POA, SDM, NOK and financial status
  • Identifies and intervenes in high-risk social situations (e.g., IPV, abuse) using best practice guidelines
  • Provides theory-based counselling including crisis intervention and illness adjustment counselling
  • Provides case management for an assigned patient area including discharge planning and linking to community services
  • Coordinates complex discharges and program-specific case management activities
  • Leads discharge planning including needs/resources for safe discharges and family involvement
  • Facilitates family meetings requiring therapeutic counselling and negotiation
  • Provides mentorship to new staff and participates in professional development
  • Meets OCWSSW re-certification requirements and completes required data collection
