Job summary

NAANDWECHIGE-GAMIG Wikwemikong Health Centre in Ontario is seeking a Social Worker to conduct psychosocial assessments and develop treatment plans in collaboration with patients, families and the health care team.

The role focuses on helping patients manage hospitalization, illness, diagnosis and life situations, including mental health concerns. Strong communication and cultural safety are essential.

Qualifications

• Bachelor's degree in social work; Master’s degree preferred.
• Registered in good standing with OCSWSSW.
• Minimum 3 years of social work experience in health care or social service setting.
• 2 years post-master’s clinical experience preferred.
• Excellent assessment, interviewing, and counselling skills.
• Knowledge of mental health and substance abuse areas.
• Certification: ASIST, Mental Health First Aid, CISM L1/L2, First Aid/CPR, WHMIS asset.
• Ability to make independent decisions and refer when needed.
• Strong communication and interpersonal skills.
• Trauma-informed and culturally safe practice knowledge.

Responsibilities

• Provide psychosocial assessment and social work treatment plans.
• Support patients and families affected by chronic, acute, or terminal illnesses.
• Advise family caregivers and plan post-discharge needs.
• Develop treatment plans engaging clients in their own care.
• Consult with doctors and other health professionals.
• Maintain treatment records and prepare reports.
• Attend department meetings and in-service programs.
• Coordinate with community agencies to ensure seamless care.
• Provide mental health counselling one-on-one or in groups.
