Job summary

ParaMed Home Health Care in Canada seeks a Clinical Planner to onboard new patients requiring Personal Support Services within an assigned district, assessing needs, implementing care plans, and evaluating interventions. You will ensure safe living environments, initiate personal care, and guide scheduling based on clinical requirements.

The role requires RPN/LPN registration with provincial bodies, 2+ years nursing experience, strong communication, and knowledge of relevant health legislation.

Qualifications

• Registration in good standing as an RPN/LPN with provincial governing bodies.
• Minimum two years’ experience as a practicing nurse.
• Working knowledge of applicable legislation including Mental Health Act, Substitute Decisions Act, Healthcare Consent Act, Privacy / PHIPA, and OHS.
• Strong oral and written communication skills; excellent organisational skills.
• Demonstrated competency in patient-centered care and community resources.
• Ability to provide a current, clear criminal background check with VPS.
• Access to a personal vehicle is required.

Responsibilities

• Perform the initial assessment of care needs for new patients and establish individual care plans for patient services.
• Complete the initial safety assessment of the patient's living environment to ensure safe and healthy work conditions.
• Perform the initial personal care services for new patients.
• Advise Operations on scheduling parameters for ongoing visits based on clinical needs.
• Inform patients and families of ParaMed policies and scheduling practices, including expected time of care.
• Maintain current and accurate patient records.
• Respect cultural needs, dignity, privacy, and confidentiality of patients and staff.
• Stay abreast of changes in practice settings that affect scope of practice.

The Clinical Planner is accountable for onboarding new patients requiring Personal Support Services within an assigned district, through the assessment, implementation, and evaluation of a wide range of clinical and personal care interventions.

What You’ll Do

• Perform the initial assessment of care needs for new patients and establishes the individual care plans for patient services.
• Complete the initial safety assessment of a new patients’ living environment to ensure employees will be working in safe and healthy work conditions.
• Perform the initial personal care services for new patients.

What You Bring

• Registration in good standing as a RPN/LPN with provincial governing bodies and minimum two years’ experience as a practicing nurse.
• Working knowledge of applicable legislation including but not limited to Mental Health Act, Substitute Decisions Act, Healthcare Consent Act, Privacy / Personal Health Information Act, Occupational Health and Safety legislation and practices.
• Knowledgeable in best practice standards for community health care and services.
• Strong oral and written communication skills

• Demonstrated competency in patient centered care principles and practices.
• Knowledgeable of community resources available for patients and families.
• Strong independent critical thinking and problem-solving skills.
• Ability to provide a current, clear criminal back check with Vulnerable Position Screening (VPS)
