Job summary

ParaMed in Ottawa is seeking a Clinical Planner to onboard new patients requiring personal support services. You will assess care needs, establish individualized care plans, and coordinate initial and ongoing visits to ensure safe and effective home health care.

Applicants must be an RPN/LPN in good standing with two years of nursing experience, with strong communication, organizational, and patient-centered care skills. A valid VPS background check and access to a personal vehicle are required.

Qualifications

  • Registration in good standing as an RPN/LPN with provincial bodies and minimum two years’ nursing experience.
  • Knowledge of Mental Health Act, Healthcare Consent Act, Privacy Act and related health legislation.
  • Strong written and verbal communication skills; organized and detail-oriented.
  • Competence in patient-centered care and ability to work with diverse patient populations.
  • Ability to obtain a clear VPS/criminal background check and access to a personal vehicle.

Responsibilities

  • Perform initial assessment of care needs for new patients and develop individual care plans.
  • Complete initial safety assessment of patients’ living environment for safe work conditions.
  • Provide initial personal care services to new patients.
  • Advise Operations on scheduling parameters for ongoing patient visits based on clinical needs.
  • Inform patients and families about ParaMed policies, scheduling practices, and expected timing of care.
  • Maintain accurate patient records and ensure privacy and confidentiality.

The Clinical Planner is accountable for onboarding new patients requiring Personal Support Services within an assigned district, through the assessment, implementation, and evaluation of a wide range of clinical and personal care interventions.

What you’ll do:

  • Perform the initial assessment of care needs for new patients and establishes the individual care plans for patient services.
  • Complete the initial safety assessment of a new patients’ living environment to ensure employees will be working in safe and healthy work conditions.
  • Perform the initial personal care services for new patients.
  • Advise Operations of the appropriate parameters for scheduling the patient’s ongoing visits, based on clinically indicated service requirements.
  • Inform patients and families of ParaMed policies and scheduling practices, as well as the expected time of care.
  • Maintain current and accurate patient records.
  • Show respect for cultural needs, dignity, privacy, and confidentiality of patients and staff.
  • Stay abreast of changes in practice setting that affect scope of practice.

What you bring:

  • Registration in good standing as a RPN/LPN with provincial governing bodies and minimum two years’ experience as a practicing nurse.
  • Working knowledge of applicable legislation including but not limited to Mental Health Act, Substitute Decisions Act, Healthcare Consent Act, Privacy / Personal Health Information Act, Occupational Health and Safety legislation and practices.
  • Knowledgeable in best practice standards for community health care and services.
  • Strong oral and written communication skills
  • Excellent organizational skills
  • Demonstrated competency in patient centered care principles and practices.
  • Knowledgeable of community resources available for patients and families.
  • Strong independent critical thinking and problem-solving skills.
  • Ability to provide a current, clear criminal back check with Vulnerable Position Screening (VPS)
  • Access to a personal vehicle required
