Job summary

ParaMed is seeking a Clinical Planner to onboard new patients requiring Personal Support Services across an assigned district. You will assess needs, implement care plans, and evaluate services while ensuring safety and privacy.

Strong nursing background and knowledge of relevant acts are essential. You will collaborate with Operations to schedule visits, inform families of policies, and maintain thorough patient records, all while upholding patient-centered care principles and cultural safety

Qualifications

  • Registered in good standing as an RPN/LPN with provincial bodies.
  • Minimum two years’ nursing experience.
  • Knowledge of Mental Health Act, Privacy Act, and OHSA.
  • Understanding of community health care standards and practices.
  • Excellent communication and organizational skills.
  • Ability to work independently with critical thinking.

Responsibilities

  • Perform initial assessment of care needs and establish individual care plans.
  • Complete initial safety assessment of patient living environment for safe work conditions.
  • Perform initial personal care services for new patients.
  • Advise operations on scheduling based on service requirements.
  • Inform patients and families about ParaMed policies and scheduling.
  • Maintain current and accurate patient records.
  • Stay updated on practice changes affecting scope of practice.
