ParaMed in Toronto, Ontario is seeking a Clinical Planner (RPN/LPN) to onboard new patients requiring Personal Support Services. You will assess needs, implement care plans, and evaluate interventions, ensuring safe environments and high-quality care.

The role requires registration as an RPN/LPN and a minimum of two years’ nursing experience, with strong communication and organizational skills. Travel may be required, and a comprehensive benefits package is offered.
