Job summary

University Health Network (UHN) is seeking a part-time Pharmacist to deliver patient-focused care at the Toronto Rehabilitation Institute and UHN Reactivation Care Centre. The role involves pharmacotherapy assessments, medication histories, and ensuring safe medication use across multiple rehabilitation programs.

The position is temporary part-time, 22.5 hours per week, with shifts Monday through Friday, including weekends and on-call.

Qualifications

• Bachelor of Science in Pharmacy or Doctor of Pharmacy degree (or equivalent)
• Pharmacy competency certificate from the Pharmacy Examination Board of Canada
• Active registration with the Ontario College of Pharmacists (Part A)
• Hospital pharmacy experience preferred; residency is an asset
• Hospital pharmacy residency certificate is an asset
• Membership in the Canadian Society of Healthcare-Systems Pharmacy an asset
• Excellent verbal and written communication skills
• Excellent interpersonal skills

Responsibilities

• Provide patient-focused care to improve outcomes and conduct pharmacotherapy assessments and medication histories
• Support operational patient care activities including drug distribution, access, procurement, and coverage
• Participate in education, professional development, mentorship and training of staff and learners
• Contribute to development, review, and evaluation of pharmacy projects and initiatives
• Provide drug information to the healthcare team to promote safe, effective, and cost-efficient therapy
• Communicate effectively with patients and the health-care team
• Perform other duties as required
