Job summary

University Health Network (UHN) is seeking a Pharmacist to provide patient-focused care to inpatient units at West Park Healthcare Centre with cross-coverage to other areas as required. The role supports a wide range of rehab and specialty units and promotes medication safety.

Responsibilities include pharmacotherapy assessments, medication histories, drug information, and collaborative work with the care team.

Qualifications

• Bachelor of Science degree in Pharmacy, Doctor of Pharmacy degree or equivalent required.
• Certificate of competency from the Pharmacy Examination Board of Canada required.
• Active registration with the Ontario College of Pharmacists (Part A) required.
• Hospital pharmacy practice experience preferred.
• Clinical experience with patient-centered care in rehab an asset.
• Hospital pharmacy residency certificate an asset.
• Membership in the Canadian Society of Healthcare-Systems Pharmacy an asset.
• Excellent verbal and written communication skills.
• Excellent interpersonal skills.
• Excellent organizational and time management skills.

Responsibilities

• Provides patient-focused care to improve patient outcomes; conduct pharmacotherapy assessments and medication histories.
• Supports operational patient care activities including drug distribution, procurement, and drug coverage; promotes medication safety.
• Participates in education activities including professional development and mentoring.
• Contributes to development, review, and evaluation of pharmacy projects and initiatives.
• Provides drug information to healthcare teams to promote safe, effective medication use.
• Communicates effectively with patients and healthcare team.
• Performs other duties consistent with the job classification.
