Job summary

University Health Network is seeking a Leukemia Coordinator to join the Acute Leukemia Program at Princess Margaret Cancer Centre. The role partners with program leadership to coordinate care for leukemia patients, including transplant pathways and rapid assessment clinic referrals.

A BSN is required, with five years of relevant experience in advanced clinical nursing and Ontario licensure. The position offers a permanent full-time status, regular Monday–Friday hours, and a comprehensive

Qualifications

  • Bachelor of Science in Nursing (BSN) required
  • Current certificate of competency (licensure) from the College of Nurses of Ontario
  • Completion of Basic Cardiac Life Support (BCLS)
  • Five years of current relevant experience in advanced clinical practice
  • Oncology Nursing certification preferred
  • Effective organizational and interpersonal skills
  • Excellent decision-making and problem solving skills
  • Excellent time management
  • Excellent teaching abilities with individuals and groups
  • Ability to work within an inter-disciplinary team or independently

Responsibilities

  • Coordination and management of care for leukemia patients within shared care and rapid assessment pathways
  • Coordinate pathways for allogeneic transplant and new patient consultations
  • Act as a nursing consultant to resolve complex patient management issues
  • Provide education to patients and staff
  • Contribute to quality improvement and research activities
  • Perform administrative duties as required
  • Collaborate across departments to support patient-centered care

Position Summary

The Leukemia Coordinator works with the Leukemia Program team & leadership, as well as other internal and external provider care teams to ensure the effective management, planning and coordination of care for Leukemia patients.

Duties

The principal focus of care will include:

  • Management and coordination for patients in shared care, transitioning and planning for allogeneic blood & marrow transplant, and new patient consultations through our Rapid Assessment Clinic.
  • The Leukemia Coordinator will utilize and apply current specialized knowledge and expertise on recent advances in nursing practice to provide, manage, coordinate and facilitate patient care for patients with complex needs both directly and through consultation
  • Functioning as a consultant in the resolution of complex patient management problems
  • Performing teaching/education responsibility
  • Collaborating with the others to develop and implement continuous quality improvement activities
  • Participating in academic and research activities/projects
  • Performing administrative responsibilities where indicated
  • Performing cross-functional and/or other duties consistent with the job classification, as assigned or requested.
  • Bachelors of Science in Nursing, Master’s Degree preferred
  • Current certificate of competency (licensure) from the College of Nurses of Ontario
  • Completion of Basic Cardiac Life Support (B.C.L.S.) program
  • Five years current and relevant experience, including experience working in an advanced clinical practice role required (e.g. experience working with malignant hematology patients)
  • Certification in Oncology Nursing from C.N.A. preferred
  • Effective organizational, interpersonal and communication skills
  • Excellent decision-making and problem solving skills
  • Excellent time management skills
  • Excellent teaching skills with the ability to teach one-on-one and/or groups
  • Ability to function collaboratively within an inter-disciplinary team and/or independently
  • Ability to work in a self-directed manner
  • Demonstrated awareness of and adherence to professional and ethical standards
  • Client service oriented, with the ability to effectively work with diversity and appreciate that people with different opinions, backgrounds and characteristics bring richness to the challenge or situation at hand
  • Computer literacy
  • Research experience preferred
  • Membership with the RNAO preferred
