GWEKWAADZIWIN MIIKAN EMPLOYMENT OPPORTUNITY

Clinical Care Navigator

Internal/External Posting

Position Description:

The Clinical Care Navigator (CCN) is responsible for supporting case management across all phases of Gwekwaadziwin Miikan programming. The CCN will work directly with the Clinical Director to ensure a high level of clinical care is administered. This position will be responsible for medication oversight, including pick-ups, transportation, and charting. This position will liaise with external agencies in a timely and professional manner to ensure appropriate flow of operations and wraparound care are maintained.

This is a full-time contract position until March 31, 2026, with the possibility of permanent employment pending successful evaluation.

Responsibilities and Duties:

  • Support program participants through collaborative treatment and wellness planning from intake to referral/program completion
  • Responsible for the coordination and oversight of medication, including medications pick-up, transportation, charting and compliance with organizational policies and procedures
  • Develop and maintain medication and administration records for clients
  • Coordinate with pharmacy for medication for clientele and prepare it for the operations of the program
  • Liaise with external agencies for the purpose of providing timely and adequate provision of care
  • Assist with development of program policies and procedures
  • Management of medication administration records via electronic medical record system
  • Other program duties as required

Qualifications:

  • Minimum requirement is a degree in social services, nursing, or other related human service diploma, with mental health and addictions training
  • Understanding of the provincial scope of First Nations mental health and addictions programming and challenges
  • Proven experience with office administration, and electronic medical records
  • Experience working in Mental Health and Addictions field
  • Preference given to an Individual of Anishinaabe ancestry with knowledge and understanding of Anishinaabek history, clan systems, protocols, etiquette, cultures, and traditions.
  • Demonstrated professional level of interpersonal and communication skills (written and verbal)
  • Ability to prioritize multiple responsibilities and projects
  • Mature individual who can work independently with preference given to individuals living a traditional lifestyle and being on a life-long journey in traditional knowledge

Requirements:

  • Possesses a valid Ontario class ‘G’ driver’s license in good standing with access to a reliable vehicle
  • Ability to work outside normal business hours as required
  • Preference for a designation with a regulatory college

Salary: Starting at $54,000/annual
