Job summary

York Hills Centre for Children, Youth and Families in Ontario is seeking a dedicated Child and Family Worker - PATH to deliver direct services to children, youth, and families. You will collaborate with schools, health services, and community partners to stabilize families facing crises and promote healthy development.

This position requires post-secondary training in Child and Youth Care or Social Work, at least two years of relevant experience, and professional registration where applicable.

Qualifications

• Completion of a post-secondary education in Child and Youth Care, Social Work or a related field.
• Minimum two years of relevant experience working with children, youth, and families.
• Registration with the Ontario College of Social Workers and Social Service Workers or College of Registered Psychotherapists of Ontario, where applicable.
• Knowledge of child and youth mental health legislation and regulations.
• Willingness to work flexible hours including evenings and in home/school/community settings.
• Excellent organizational, communication, and time-management skills.
• Ability to work independently and in teams, handle crises, and maintain confidentiality.

POSITION

Reporting to the Program Coordinator - PATH (Planning Alternative Tomorrows with Hope), the Child and Family Worker - PATH is responsible for providing direct service to children, youth, and their families. This role is pivotal in addressing a range of mental health issues including emotional, behavioural and/or psychiatric symptoms that may be impacting functioning at home, school and/or in the community. The Child and Family Worker - PATH works with clients to stabilize and strengthen families who have experienced recent crisis in their lives. This role will collaborate with various staff and departments, including upholding the agency’s Mission, Vision and Values while fostering a positive workplace environment.

A LOOK AT YOUR DUTIES AND RESPONSIBILITIES:

Client Service Responsibilities

• Assists in promoting and supporting the Mission, Vision, and Values statements, ensuring adherence to operational and service policies and procedures while maintaining ethical conduct.
• Demonstrates a strong understanding of ethical behaviour and professional conduct, including contributing to ensuring ethical standards are upheld.
• Navigates complex client needs while building, maintaining, and nurturing key stakeholder relationships within the child welfare, education, and hospital sectors in York Region.
• Manages complex and challenging situations within families working towards stabilization goals.
• Utilizes risk assessment strategies and implements suicide prevention and/or intervention techniques.
• Develops and implements stabilization plans with children, youth and their families/caregivers using a client-centered, strength-based and trauma-informed approach.
• Utilizes a variety of evidence-based/informed approaches to enhance parent/family/ child/youth interactions and functioning at home, in school and in the community.
• Provides case management for clients with multiple and complex needs, which includes referrals to internal and/or external programs.
• Works collaboratively with external systems such as schools and Children’s Aid Societies and attends meetings as needed.
• Works collaboratively within a multidisciplinary team, to ensure a positive teamwork environment when planning around shared clients and the coordination of services between programs.
• Facilitates workshops and/or groups for diverse client populations.
• Identifies clinical learning needs and seeks consultation and support through supervision.

Administrative and Organizational Responsibilities

• Ensures client records are completed within scheduled timeframes and program demands, including timely report submissions.
• Maintains client records in accordance with quality assurance policies and procedures, including accreditation standards and licensing requirements.
• Works with Human Resources to ensure memberships, licenses, background checks and certifications are up to date via submission of required documents on ADP.
• Builds and maintains interactions with colleagues focused on individual, team, and organizational success.
• Performs work in a safe manner and adheres to the established health and safety measures of the organization.
• Actively supports Equity, Diversity and Inclusion drivers, including fostering a welcoming and safe space, where people can expect to be treated fairly, equitably and in a culturally responsive way.
• Attends and participates in required training and meetings.
• Other duties as assigned.

WHAT WE ARE LOOKING FOR:

• Completion of a post-secondary education in Child and Youth Care, Social Work or a related field and/or equivalent combination of education and experience.
• Minimum of two (2) years of relevant experience working with children, youth, and families.
• Registered with the Ontario College of Social Workers and Social Services Workers and/or College of Registered Psychotherapists of Ontario, etc. where applicable
• Knowledge of child and youth mental health legislation and regulations.
• A willingness to work flexible hours which includes 2 evenings per week; comfortable working in a client's home, the community or school setting as needed.
• Excellent interpersonal, verbal, and written communication skills, including editing skills with attention to detail.
• Competence in modelling professionalism and maintaining a high level of confidentiality.
• Excellent organizational, solution-based problem-solving and time-management skills, with the ability to manage multiple priorities and meet deadlines.
• Demonstrates versatility in the ability to work independently and collaboratively to support agency and program goals.
• Ability to work under pressure and in crisis situations.
• Demonstrates sound judgement related to the development of collaborative approaches working with children, youth, and their families, including high-risk circumstances and safety planning.
• Ability to identify learning needs, seek consultation, and support through supervision.
• Experience facilitating groups, webinars, and workshops.
• Excellent organizational, planning, and time management skills.
• Functional technological skills (e.g., Office 365, MS Office, SharePoint, client information systems, and software applications).
• Awareness and appreciation for the dynamics of equity issues (diversity, equity, and inclusion) especially as it relates to gender, ethnicity, race, and economic status.
• Identification with the strategic mission, vision and values of the organization including inclusion, collaboration, people centred, knowledgeable and courageous.
• Driver's license and clear driver's abstract, including proof of automobile insurance.
• Must possess a satisfactory Police Record Check.
