Job Description

In accordance with the Vision, Purpose, and Values, and strategic direction of Island Health (Vancouver Island Health Authority), patient and staff safety is a priority and a shared responsibility. The requirement to continuously improve quality and safety is inherent in all aspects of this position.

Under the administrative direction of the Team Lead, and as part of the therapeutic team under the Coordinator of Access and Crisis Services with Island Health, the Case Worker is responsible for providing primary therapeutic, case management, and mental health support services for assigned clients in hospital, residential, or community settings. Services are provided according to care plans agreed upon by the multidisciplinary clinical team, the individual, and family.

Qualifications:

• Baccalaureate degree in a mental health-related discipline, with eligibility for licensing or registration in a relevant professional association, plus two years of recent related experience, or an equivalent combination of education, training, and experience.

Skills and Abilities:

• Clinical understanding of substance use, affective disorders, schizophrenia, and other psychiatric disorders, using a biopsychosocial care model.
• Ability to work in outreach case management.
• Effective team collaboration skills.
• Ability to manage diverse community and facility contacts, including patients, families, staff, physicians, psychiatrists, and community agencies.
• Effective organizational and prioritization skills.
• Strong verbal and written communication skills.
• Possession of a valid BC Driver's License.
