Job Description

In accordance with the Vision, Purpose, and Values, and strategic direction of Island Health (Vancouver Island Health Authority), patient and staff safety is a priority and a shared responsibility. Continuous improvement of quality and safety is inherent in all aspects of this position.

Under the administrative direction of the Coordinator and as part of the therapeutic team under the Clinical Director, the Case Worker provides primary therapeutic, case management, and mental health support services for assigned clients in hospital, residential, or community settings. Services are provided as needed, in accordance with care plans developed by the multidisciplinary clinical team, the individual, and family.

Qualifications :

• Baccalaureate degree in a mental health-related discipline.
• Eligibility for licensing or registration with a relevant professional association.
• Two years of recent related experience or an equivalent combination of education, training, and experience.

Skills and Abilities :

• Clinical understanding of affective disorders, schizophrenia, and other serious psychiatric disorders, using a biopsychosocial care model.
• Ability to work in outreach case management.
• Effective team collaboration skills.
• Ability to communicate effectively verbally and in writing.
• Valid Driver's License.
• Ability to manage work priorities and meet deadlines.
• Ability to engage effectively with patients, families, staff, and community agencies.
